Required Information
1. Complete Acord Applications.
2. Complete First Capital Supplemental Application.
3. Minimum of 3 years plus current year loss experience valued within
90
days.
4. Complete vehicle schedule with classes and weights.
5. List of drivers.
6. Copy of recent MVRs
7. Depending on size of fleet (Vehicle maintenance procedures - Hiring
guidelines, Insured policy on personal use of autos may be required.)
